 Services Program -  Care Coordinator Program 
Purpose

The intent of the Kentucky HIV/AIDS Care Coordinator Program (KHCCP) is to facilitate the provision of quality care and services to HIV-infected individuals and their families in a timely, consistent manner of continued care. The program provides care coordinators in six regional sites through arrangements with two local health departments in the Cumberland Valley and Northern Kentucky regions and four non-profit agencies in the Barren River, Lexington, Louisville and Purchase regions to assess needs and provide services. These regional sites allow for statewide coverage and better local access to these services.

KHCCP also is an umbrella program for other client assistance programs such as the Kentucky Health Insurance Continuation Program, Outpatient Health Care and Support Services and state support service programs. KHCCP receives federal funds and is a service-funding source of last resort. Financial assistance is not guaranteed. Funding is limited and services may be terminated without cause. Services in each region may vary depending on funding. Continuation of all programs is contingent upon availability of state and federal funding.

Goals
* To optimize client self-care capabilities by supporting self-sufficiency.
* To identify and facilitate coordination between clients' informal support systems (family, friends, care giver, group sponsor, psychotherapist, etc.) and available formal support systems.
* To help clients locate and access system of referrals to medical care, housing, counseling, Medicaid, Social Security Disability, transportation and legal services.
* To minimize or eliminate duplication of services among formal and informal support systems.
* To provide clients with educational information regarding disease transmission and health maintenance, encourage good health habits and provide secondary prevention through continued case management.
* To identify and document patterns of  service needs and advocate for effective policies and resource development.
* To ensure program funds are used efficiently and effectively to meet the needs of HIV/AIDS-infected people through coordinated funding streams and use of existing community resources and services.

 Financial Assistance Programs 

Basic Eligibility Criteria for Financial Assistance Programs
Household income - no more than 300 percent or of federal poverty level
Household resources - case assets less than $10,000
Residency - must be a resident of Kentucky
Medical documentation - HIV positive status must be confirmed with appropriate documentation (for KADAP participation, medical documentation must also include CD4+ T cell count and viral load)
Lack of other third-party payer - must be ineligible for assistance from other third-party payers for assistance requested

Kentucky AIDS Drug Assistance Program (KADAP)
This program helps eligible low-income Kentuckians purchase AIDS-related, FDA-approved medications. Participants receive formulary medications through mail-order service provided by the Kentucky Clinic Pharmacy in Lexington. For further information call 1-866-510-0005

Kentucky Health Insurance Continuation Program (KHICP)
KHICP provides payment to continue health insurance coverage for eligible individuals at risk of losing employee health benefits or private-pay health insurance because of HIV-related disease.
Eligible for the Health Insurance Program participants must:
Have a Medicare Part D or a prescription rider as part of a health insurance policy. Individual policies must cover HIV positive status. Family policies must cover at least two HIV positive family members
Part B funds may be used, if cost effective, to pay premiums through an insurance continuation program if the participant can't afford the costs and meets state income eligibility criteria

Kentucky Outpatient Health Care and Support Services Program
These programs provide a wide range of community-based medical and non-medical support services, including physical and mental health care, housing, nutrition and transportation services. Eligible priority services are identified during each funding period based on client and care coordinator input, needs assessments, resource inventories, client satisfaction surveys, funding limitations and other factors.

Patient Assistance Programs for HIV Treatment

Accessing medications that you need can be difficult, but there are often programs such as our federally funded KADAP, various Patient Assistance Programs or medication assistance programs that can help. The Access Project of the AIDS Treatment Data Network can help you learn about these programs, as well as maximize your benefits through Medicare, Medicaid, or any type of private insurance you might have. The goal is to reduce your expenses to the smallest amount possible, and to make sure you know you do have options.

Kentucky HIV/AIDS Planning and Advisory Council

The Kentucky HIV/AIDS Planning and Advisory Council (KHPAC) is a 30-member body appointed by the governor to carry out the provisions of KRS 214.640, the Centers for Disease Control and Prevention HIV Prevention Community Planning Guidance and the Health Resources and Services Administration Planning Bodies Manual. KHPAC works in collaboration with the HIV/AIDS Branch of the Kentucky Department for Public Health to ensure HIV/AIDS prevention activities, services and  policies meet the needs of the people of the Commonwealth of Kentucky.

Membership - Council representation and structure
KHPAC consists of the commissioners of the departments for Public Health and Medicaid Services and 28 appointed members who represent HIV/AIDS stakeholders, including people living with HIV/AIDS, high-risk populations, AIDS service providers, HIV/AIDS prevention workers, mental health providers, community-based organizations and friends and family of people living with HIV/AIDS.

HIV/AIDS Case Management

HIV/AIDS case management services provide support, stabilization and referrals to community resources for persons living with HIV/AIDS and members of their households. Services are available to those living in the counties of Boone, Campbell, Carroll, Grant, Gallatin, Kenton, Owen and Pendleton. Services include: Short-term crisis counseling, housing and utility assistance, assistance with drugs and medicine, dental care, health insurance, transportation, nutritional guidance, links to support groups and volunteers, assistance in obtaining a medical doctor, and referrals for specialized counseling, food, housing and other community services. HIV/AIDS Outreach Services
Outreach services are provided to the community to promote HIV testing and counseling, and to encourage HIV Positive individuals to seek case management services. Special emphasis is on the hard to reach high-risk groups, such as African-Americans, Hispanics, substance abusers, homeless persons and the incarcerated. For more information, call 859.341.4264.

HIV/AIDS/STI Prevention Education
HIV/AIDS Prevention and Education programs are available to community, professional groups, and businesses upon request. Risk reduction education is also provided to high-risk groups. For more information, please call 859.341.4264.

HIV/AIDS Professional Education
An HIV/AIDS Continuing Education Update course, approved by the Kentucky Cabinet for Health and Family Services, is a multidisciplinary class suitable for any of the professional groups impacted by KRS 214.610/615. It is a 2-hour course with a cost of $20, payable on the date of the class. The course is offered bi-monthly.
